  • You must be an active member of Philhealth, meaning you have paid at least three monthly contributions within the immediate six months before the date of confinement or availment



  • You must be confined or availed of services in an accredited hospital, facility, or health professional

How to fill out and submit Philhealth Form PMRF?

The first part of Philhealth Form PMRF is where you will enter your personal details, such as your name, date of birth, gender, marital status, and nationality.   • Select your gender from the drop-down list. You can choose between Male or Female.

  • Select your nationality from the drop-down list. You can choose from a list of countries or regions.

The third part of Philhealth Form PMRF is where you will declare your dependents, who are the people who can avail of benefits under your membership. You can declare up to four dependents, such as your spouse, children, parents, or siblings. You need to make sure that they meet the eligibility criteria and that you have the necessary documents to prove their relationship to you.


To declare your dependents, you need to follow these steps:



  • Enter the name, relationship, date of birth, and gender of each dependent in the corresponding cells. Use capital letters and avoid abbreviations or nicknames.



  • Select Yes or No from the drop-down list to indicate if the dependent is employed or not.



  • Select Yes or No from the drop-down list to indicate if the dependent has a disability or not.



  • Select Yes or No from the drop-down list to indicate if the dependent is a senior citizen or not.



Here are some of the eligibility criteria and documentary requirements for dependents:



  • Your spouse must be legally married to you and not a member of Philhealth. You need to submit a photocopy of your marriage certificate.



  • Your children must be below 21 years old, unmarried, unemployed, and not a member of Philhealth. You need to submit a photocopy of their birth certificates.



  • Your parents must be 60 years old and above, not a member of Philhealth or a dependent of another member. You need to submit a photocopy of their birth certificates or senior citizen IDs.



  • Your siblings must be below 21 years old, unmarried, unemployed, not a member of Philhealth or a dependent of another member, and have a congenital disability. You need to submit a photocopy of their birth certificates and medical certificates.



Philhealth Form PMRF updating/amendment




The fourth part of Philhealth Form PMRF is where you will indicate if you want to update or amend your membership information. You may need to do this if there are any changes in your personal or membership details, such as your name, address, contact number, marital status, dependents, etc. You need to make sure that you inform Philhealth about these changes as soon as possible, so that your records are updated and your benefits are not affected.
